If a coach enters the field of play, during the game, without the referee's permission, should the referee caution and show a yellow card to the coach or should he advise the coach that the referee's permission is required to enter the field of play, and report the incident to the proper league authority?

In games played under FIFA and/or USSF (including AYSO) jurisdiction, coaches are never shown cards, cautioned, or sent off. If their behavior is unacceptable they may be warned or dismissed from the field according to the referee's determination of the seriousness of their misconduct. In any event, their behavior, and the referee's response to it, must be included in the game report.

At this age group I would first advise the coach and then after repeated attempts tell him that the game would be abandoned but I would rule out asking him to leave and report the incident to the proper authorithies.
