Stoppage time is added for all lost time in a half, at the discretion of the referee. I haven't seen lots of stoppage time added, maybe three or four minutes. Are there recommended limits for stoppage time, or any informal "best practices" that limit stoppage time to a small number of minutes?

At the world cup
injury when a player is hurt badly that he lies for an extended time on the pitch AFTER play is stopped.
Because of available safety personal this is not as much as in youth or recreational matches

Each Substitution is aprox 30 seconds .
This differs in unlimited substitution matches as in youth.

referee discrestion as to wasteing time but they are quick to caution to make up for it.

Per Law 7 The Duration of The Match
Allowance is made in either period for all time lost through:

substitution(s);

assessment of injury to players; removal of injured players from the field of play for treatment;

wasting time;

any other cause.

The allowance for time lost is at the discretion of the referee.

Stoppage time id added at the discretion of the referee and amounts to all the time he thinks is lost. One needs only to look at the Argentina 6:0 Serbia/Montenegro match, on 15 June 06, to see what the referee's discretion can be, the match stopped on 90 minutes with no additional time added.
