At least one, per FIFA.

Leagues may, and often do, specify longer suspensions. For particularly grevious offenses, such as assault, the player may be banned for life.

That is between the league and the referees report. It all depends. Different leagues are different. It is important that the referee document everything and is very clear on what happend.
