How far off the pitch can a flip throw in take a run from? Also, how far can a defender stand from where the throw in is taken place?

As long as the flip thrower takes the actual throw from within a yard of where it went out, it doesn't really matter how long the run up is - with the caveat that too long a run could be considered a delay of restart. That should elicit a warning first from the referee and a caution upon repeat, although they could legally go straight to caution (not advised). As for the distance the defender must stand, FIFA made a Law change a couple of years ago which says the defenders must be a minimum of 2 yards away from the point where the throw-in is taken. And just a reminder that the Law has told us for many, many years that defenders are not allowed to interfere with the throw-in, and doing so will garner them a caution.
