Throw-in in your favor [your team only]
Goal kick [both teams]
Kick off [both teams]
Injury [both teams]
The interval between the two periods of play [both teams]
A caution [the cautioned player only].
But the "Official Modified Under 12 Playing Rules" state that substitutions are allowed "Substitutions: At any stoppage and unlimited."
So where do the criteria for only on throw-in your possession, goal kick, kick off, injury, etc come from? Answer provided by Referee MrRef Here is where the answer was quoted:

I have read what USYSA playing U-12 recommendations read. I offered what referees are taught and unless referees are told something else this is when substitutions are going to occur.
So there is a discrepancy someplace, clarification is necessary.
US Soccer offers this:
Here is what the latest USYSA rules for players and Playing Rules. Policy 12-1-06, has to say:
Rule 302. SUBSTITUTIONS Section 1. Except as provided by USYSA or its State Associations, substitutions shall be unlimited except where specified otherwise in the rules and regulations for a special competition. Section 2. Substitutions may be made, with the consent of the referee, at any stoppage in play.
