DOES THE BALL HAVE TO BE IN PLAY BEFORE THE REFEREE CAN BLOW THE FINAL WHISTLE?

No. The game is over when the referee decides it is and it doesn't matter if the ball is in play or not

No. Though it does seem to be the case in my playstation 3 video game of FIFA '08. In real life, the ball doesn't need to be in play.

No! Although it is advisable to end play so the ball is nearby for a referee to retrieve. (o:
