No. This is one of the enduring myths of the game. The referee will keep track of which players have taken kicks.

No, and they don't have to repeat the order on the second round of kicks, should it get that far. The only restriction is no kicker can take a second kick until every team member in the rotation has taken a kick. So, let's say your team had 11 kickers and the other team has ten, because one of them drew up lame after the kicks commenced. All 11 of your team's kickers must kick before starting over, even though the team of 10 will start over sooner in their rotation.

No, there are five penalty takers from each side unless it is impossible for one side to score an equal number of goals than their opponents. Once five penalty takers have had their turn alternating players will take a penalty until one side or another runs out winners. Before a player may take a second penalty all of his team mates must have taken their turns.
