Answer provided by Referee Michelle Maloney

All of your questions are answered by the Laws of the Game, the Advice to Referees and the Instructions following the 2008/2009 body of the LOTG.

1 & 4)A player from a team may change places with the goalkeeper at any stoppage in a game with the referee's permission. Law 3
Because the LOTG still apply during the KFTPM, a player may still change places with the GK with the referee's permission. LOTG - Procedures to Determine the Winner of a Match

2, 3 & 4) If the keeper is injured while kicks are being taken and cannot continue, and (big AND) the team has a substitution left, the keeper may be replaced. LOTG - Procedures to Determine the Winner of a Match. So, the keeper at the end of regulation or overtime will remain the keeper for the kicks, unless or until he is replaced by a field player and/or he becomes injured and can be substituted.
