When is a penalty deemed dead in a penalty shoot out...?

When the referee says it is. Law 14: 'The referee decides when a penalty kick has been completed.'

A good referee will decide the kick is over when the ball has no chance of going into the goal.

The referee should wait until the ball no longer has any momentum which could carry it into the goal before calling the kick complete. The ball can hit any combination of the crossbar, goalpost, ground or keeper and rebound, deflect or backspin into the goal from the kick and be considered good.

A penalty is completed when the referee says so. So examples of when a penalty would be complete is when the ball is kicked wide, rebounds back into the field of play with no hope of entering the goal, looses it momentum from the original kick and caught by the goalkeeper and thrown away.
