If the referee is ever knocked unconcious, or, for that matter, ever passes out for any reason in the middle of play, when exactly does play stop, and if the referee cannot continue, who makes up for the missing official when an AR takes his place?

In the pregame the referee designates one of the assistant referees to take over for him if for some reason he cannot continue.

If a referee is knocked unconscious play stops immediately

Hi
If the referee is incapacitated for whatever reason play stops immediately.
If the referee is unable to continue a decision will be made as to who will take over the CR duties. At the highest level the 4th official takes over. At lower levels the senior AR takes over and a replacement AR is found.
Depending on the status of the replacement AR a decision will be made as to his duties for the remainder of the game. If he is a club official his duties will be ball in and out of play decisions only.
