A player came from an offside position to retrieve a ball kicked from her own teammate. No one else touched the ball until she gathered it 10 yards in her own half. Offside? If so, where does the AR flag the offence?

Yes. If the player was in offside position when the ball was kicked, it doesn't matter where the player was located when she played the ball.

The AR should remain even with the second last defender and raise the flag when the attacking team player touches the ball. The restart is where the attacking player was located when the ball originally was played by her teammate.

As described this is offside and the indirect free kick is taken from the location the player was in an offside position when the ball was touched / played by a team mate not from where she gathered the ball.
