Daniel of Jupiter, Florida USA asks...

Just a quick question. If you give a yellow card i know you mark the player number and time it occurred and for what offense. The thing I'm unsure of is what you do with that info after the game is over.

Hi Daniel
You must send details of all disciplinary cards to the competition organisers along with the match card. Some Leagues have an online system which allows the referees to record the sanction/s taken while others simply look for a written form to be completed detailing the cards issued.
Perhaps some competitions such as Underage Blitz type do not require cards to be accumulated so no record is taken. However any player that has been sent off requires a report to be made in writing to the competition organisers or an affiliated League.

You need to report misconduct. How that happens is often a local implementation. Some leagues may have websites where you put in a report. Others may have you indicate it on a game sheet, with explanations required for some or all misconduct. (In my local travel league you only mark a code for cautions, but must mark a code and provide a description for send-offs.)

If you don't know what the method is in your area, the first person to ask would be the one who assigned you to the game.
