The mnemonic we use for recording is 'TLC', for trail, lead, center.

Hi Referee Steve
A nice way to remember this. Ultimately its the referee's responsibility to record the match details and he is assisted in this. Even with these recording systems mistakes continue to happen. In a PL game the 4th official asked the referee to check his caution details as he thought a player had got a 2nd caution. On checking it with the 4th official the ref was correct to chants of ' You don't know what your doing'
